Child protection forms

If you're involved with child-related ministries (including creche), you need to complete a Prohibited Employment Declaration (Form 4) and return it to one of the Pastors or the Legal Compliance Officer. Download the form here (links to NSW Commission for Children and Young People website).

SRE teachers also need to complete this form (links to Baptist Union website).
