Child protection forms

If you're involved with child-related ministries (including creche), you need to complete a Volunteer Declaration and return it to one of the Pastors or the Legal Compliance Officer. Download the form here (links to NSW Commission for Children and Young People website). If you have previously signed an Attachment 4 Prohibited Employment Declaration there is no need for you to sign this new form.

SRE teachers also need to complete an additional form - the 2012 form is here.

Incident report form

This form is to be completed whenever an incident requiring first aid or secondary medical attention occurs. It should also be completed where church property is damaged.

The completed form should be given to one of the Pastors or the Legal Compliance Officer as soon as possible after the incident. Hardcopies are also available from the noticeboard in the hall kitchen.
